1. Email - specifically as used on your Grad App

2. Date of Birth (MM/DD/YYYY)

3. You may be required to enter the phonetic spelling of your name and to speak your name. Please use a device with a working microphone. If you need assistance with the phonetic spelling, there are free tools available online.

4. To receive photo proofs from this event, provide a waist-length or head & shoulders portrait of yourself appearing similarly to how you will be on graduation day. Cap and gown photo encouraged. The image should be PNG or JPG, vertical, good resolution 400x600px or larger, preferably color. Non-copyrighted images and nice Selfies are acceptable.
